Create session hosts

Adds session hosts to an existing host pool using persisted VM settings from
PUT /api/v1/cloud/host-pools/{hostPoolId}/vm-settings.

For pools with a DynamicHosts scaling profile, scale-in cleanup treats all stopped
zero-session hosts above MinHosts as elastic capacity, including manually added hosts.

400Bad request. Invalid payload or precondition failed.

401Unauthorized. User is not authenticated.

403Forbidden. User does not have create host pool permission.

404Host pool was not found.

409Host creation is already in progress, or planned VM names already exist.
